Indication*
Hypertension; may be used alone or in combination with other antihypertensives. Heart failure in patients with impaired left ventricle systolic function (left ventrcular ejection fraction <40%) to reduce cardiovascular death and to reduce heart failure hospitalizations.
Directions for Use*
Individualized dosage. Heart failure Initially 4mg once daily, increased at approximately 2 week intervals to achieve target dose of 32 mg once daily. HTN Monotherapy in patient who is not vol depleted Initially 16mg once daily. May be administered once daily or bid with total daily doses from 8-32 mg. Patient with intravascular vol depletion Initially 4mg once daily. Renal impairment Mild: 4mg once daily. Moderate; 2 mg once daily. Hepatic impairment Mild to moderate: 2 mg once daily. Elderly Consider lower dose. General Dosing Recommendations: Individualize dose on patient's requirements.
