Brexpiprazole (Rexulti) is indicated for: Adjunctive treatment of major depressive disorder (MDD) [see Phamacology: Pharmacodynamics: Clinical Studies: Adjunctive Treatment of Major Depressive Disorder under Actions). Treatment of agitation associated with dementia due to Alzheimer's disease. Limitations of use: Brexpiprazole (Rexulti) is not indicated as an as needed (''prn'') treatment for agitation associated with dementia due to Alzheimer's disease [see Pharmacology: Pharmacodynamics: Clinical Studies: Agitations Associated with Dementia Due to Alzheimer's Disease under Actions].

Schizophrenia Initially 1mg once daily on days 1-4, titrated to 2mg once daily on days 5-7, & can be increased to max of 4mg daily on day 8. Agitation associated with dementia due to Alzheimer's disease Initially 0.5mg once daily on days 1-7, increased to 1 mg once daily on days 8-14, & to 2mg once daily on day 15. Dose can be increased to max of 3mg once daily after at least 14 days. Adjunctive treatment of MDD Initially 500 mcg or 1 mg once daily, titrated at weekly intervals to 1 mg once daily, then to target dosage of 2 mg once daily. Max: 3mg daily. Moderate to severe hepatic impairment (Child-Pugh score >7) & patient with CrCI <60mL/min Max: 2mg once daily in patients with MDD or agitation associated with dementia due to Alzheimer's disease, & 3mg once daily in patient with schizophrenia.

May be taken with or without food.
